Participating in the NLS Alumni Association is easy, fun, and rewarding--but sometimes there is paperwork! Below is a library of the various forms you might need to download and print out to participate in Alumni activities or business.
Reunions
There are two forms alumni need to bring to every reunion; these are liability waiver and medical history forms. To download and print out the forms, click on the links below.
Reimbursements
In need of reimbursement for a recent alumni or club activity, or have money from a fundraiser to deposit? Print out the Monetary Action Form below. Send the form in along with all receipts (must be included for reimbursement) to the Alumni Treasurer.
